DESCRIPTION:
Our Medicare Population Health Programs team is a centralized resource that supports care team and practice operations within Amazon One Medical Seniors. As a member of the Medicare Population Health Programs Team, the Population Health Coordinator will work to support our quality strategy by targeting outreach to patients to close gaps, manage data from multiple payers as it relates to quality, implement medication adherence standard work and/or support operations to improve the health outcomes for our patients. This is an exciting opportunity for someone looking to play a key role in a high growth, innovative healthcare company. This role reports into the Manager of Medicare Population Health Programs and will be located in an Amazon One Medical Seniors market.
Key job responsibilities
- Support a broad range of outreach for patients supporting the Medicare Population Health Programs team (this includes, but is not limited to outreach for quality, population health screenings, and more)
- Conduct targeted outreach to patients to close quality gaps for multiple measures, as informed by the quality strategy and market performance with specific targets
- Implement targeted Medication Adherence programs for at-risk patients for multiple markets, including chart review and direct outreach
- Consistently meet or exceed key performance indicator (KPI) goals
- Supports Medicare Population Health Program Needs, including chart reviews and data entry for Population Health Screening Programs
- Manual gap closure through portals and application of annotations, suspects or subscribed marker sets to support population health initiatives.
- Chart reviews as directed to support our population health strategy
BASIC QUALIFICATIONS:
- Experience with data analysis and basic Excel skills
- Experience completing complex tasks quickly with little to no guidance and react with appropriate urgency to situations that require a quick turnaround, or experience in a fast-paced, high-tech company
- Experience communicating and delivering presentations to customers, stakeholders, and/or teammates
- Experience working independently and collaborating effectively in a remote team environment
- 1+ year of experience working as a Health Coach, Lab Service Specialist, or Member Support Specialist is required
- Knowledge of Medicare quality measures
- Interest in learning how to interpret clinical data and its connection to day to day care.
