Landmark study poises to transform HRT practices for transgender women

A groundbreaking study published in the Journal of the Endocrine Society suggests a significant shift in the landscape of hormone replacement therapy (HRT) for transgender women. The research indicates that lower doses of injectable estradiol can effectively suppress testosterone while maintaining healthy estrogen levels, potentially reducing or eliminating the need for anti-androgen medications like spironolactone.

The study, which analyzed data from 2017 to 2023, tracked 29 patients who transitioned from traditional estradiol treatments (gels or tablets) to weekly estradiol injections. Over a 15-month period, researchers observed that all participants experienced a notable decrease in testosterone levels, despite a reduced average weekly estradiol dose (from 4.3 to 3.7mg). Importantly, estrogen levels remained stable.

The findings revealed that spironolactone, a commonly prescribed anti-androgen, did not correlate with lower initial testosterone levels during treatment. Furthermore, the study suggested that spironolactone could potentially lead to decreased estradiol levels compared to individuals solely on injectable estradiol.

Beyond its potential impact on hormone levels, spironolactone has been linked to side effects such as fatigue, dizziness, and increased urination. In rare cases, more severe complications like hyperkalemia, electrolyte imbalances, and kidney or liver issues have been reported.

These findings could revolutionize HRT prescription practices, potentially making estradiol monotherapy a more prevalent option. In the UK, triptorelin (Decapeptyl), an injectable testosterone blocker with a more favorable side effect profile, is increasingly replacing spironolactone. Triptorelin, typically administered every three months, offers potent testosterone suppression with less severe side effects, such as hot flashes and occasional mood fluctuations. This research paves the way for more effective and personalized HRT regimens for transgender women, prioritizing well-being and minimizing potential adverse effects.
