Scotland is facing a growing backlog for NHS specialist gender services, with Public Health Scotland reporting that 6,435 people were awaiting a first appointment at the end of March. Most waits exceeded two years, and over 1,000 patients had been waiting more than five years.
The dataset, first collected in 2023, indicates 84% of referrals waited more than a year for specialist assessment, and 4,163 people had waits beyond two years. Young adults account for a large share of demand: just over 40% of referrals in 2025/26 were for those aged 18–24, and more than 20% were for people aged 25–34, BBC reports.
Scotland’s five specialist adult gender clinics have experienced sustained pressure amid rising referrals from people questioning their gender or experiencing gender dysphoria. Referrals to the Sandyford clinic, the country’s dedicated service for children, fell by 46% last year following major service reconfiguration ordered after the 2024 independent review of care for children and young people. That review recommended delivering under‑18 care within age‑appropriate paediatric services across the country and advised pausing routine use of puberty blockers pending further research. A trial led by King’s College London will examine the impact of those medications on physical, social and emotional wellbeing.
The Scottish government acknowledges that access remains “unacceptably long” and says it is providing recurring funding to health boards to expand multidisciplinary teams, recruit staff, improve patient communications and develop new pathways and resources for those on waiting lists. Officials say work is underway with the NHS to create a more regionally distributed model for young people’s gender identity services to better support young people and their families.
